Terms of Service

Effective date: March 27, 2026 · Last updated: March 27, 2026

These Terms of Service ("Terms") govern your use of lexify.health and related services operated by Lexify Health ("Lexify", "we", "our", or "us"). By using Lexify, you agree to these Terms. If you do not agree, do not use the service.

1. Description of Service

Lexify is a Clinical Policy Intelligence platform that automatically translates clinical and medical policy documents into 7 production-ready outputs: SQL, logic specifications, test cases, QC reports, Python wrapper code, structured policy metadata, and plain-language narrative summaries — across multiple SQL dialects. The service is provided via lexify.health.

2. Acceptable Use

You agree to use Lexify only for lawful purposes and in accordance with these Terms. You agree not to:

3. No PHI — Critical Requirement

Lexify is strictly designed for clinical policy text — rules, thresholds, criteria, and logic — only. You must not input Protected Health Information (PHI) or any actual patient data. Lexify is not a HIPAA-covered entity and does not have a Business Associate Agreement (BAA) in place for standard use. Enterprise customers requiring a BAA should contact hello@lexify.health.

4. Outputs and Clinical Responsibility

Lexify generates SQL, logic specifications, test cases, QC reports, Python code, structured policy metadata, and narrative summaries based on policy text you provide. These outputs are provided as a starting point for clinical data professionals and require review by qualified clinical informatics staff before deployment in any production system.

Lexify outputs are designed to accelerate clinical data engineering work and require review by qualified clinical informatics staff before deployment in any production system. You are solely responsible for validating all generated outputs against your organization's clinical standards, data models, and regulatory requirements before use.

5. Intellectual Property

The Lexify platform, including its interface, prompts, generation logic, and brand, is owned by Lexify Health and protected by applicable intellectual property laws.

Outputs generated by Lexify from your policy inputs belong to you. You retain full ownership of generated SQL, logic specs, test cases, QC reports, Python code, policy metadata, and narrative summaries.

6. Disclaimer of Warranties

Lexify is provided "as is" and "as available" without warranties of any kind, either express or implied. We do not warrant that the service will be uninterrupted, error-free, or that generated outputs will be clinically accurate, complete, or suitable for any particular purpose.

7. Limitation of Liability

To the maximum extent permitted by law, Lexify Health shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the service or reliance on generated outputs, even if advised of the possibility of such damages.

8. Service Availability

We make reasonable efforts to keep Lexify available but do not guarantee uptime. We reserve the right to modify, suspend, or discontinue the service at any time with or without notice.

9. Rate Limits and Fair Use

Lexify enforces rate limits to ensure fair access for all users. Attempts to circumvent rate limits or use the service in ways that degrade performance for other users may result in temporary or permanent suspension of access.

10. Changes to Terms

We may update these Terms from time to time. We will update the "Last updated" date when we do. Continued use of Lexify after changes constitutes acceptance of the updated Terms.

11. Governing Law

These Terms are governed by the laws of the State of Minnesota, United States, without regard to conflict of law principles.
